طلب إعانة مادية من مؤسسة عيد الخيرية

هذا النموذج للتقديم الأولي لطلبات المساعدة المالية والعينية والغذائية..
مع أهمية تسليم الأوراق المطلوبة باليد للباحث الاجتماعي (علمًا بأنه ستتم المطابقة بين المعلومات في النموذج وما سيتم تسليمه للباحث)

– الأوراق المطلوبة للمساعدات المالية : يرجى زيارة المركز والتواصل مع الباحث الاجتماعي .

– الأوراق المطلوبة :
للأسر فقط (مساعدات غذائية وعينية):
1. صورة البطاقة الشخصية لمقدم الطلب وعائلته ( أو صورة الجواز والتأشيرة للزيارة).
2. شهادة الراتب. (حديثة).
3. صورة من عقد الإيجار (ممدة).

للفئة العمالية: مساعدة سفر :
1. صورة البطاقة الشخصية.
2. تذكرة السفر.
*المهن الأخرى: يضاف للأوراق شهادة الراتب (يشترط ألا يزيد الراتب عن 2500ر.ق).

للطالب المبتعث:
1. صورة من البطاقة الشخصية (يشترط الكفالة على الجهة التي يدرس عندها)

The papers required for food aid, furniture and electrical appliances ..:
1. A copy of the identity card of the applicant and his family (or a copy of the passport and visa for non-residents).
2. Salary certificate. (Modern).
3. A copy of the lease contract (extended).

For Labor category: travel assistance:
1. A copy of the ID card.
2. A travel ticket.
* Other professions: Salary certificate is added to the papers (the salary must not be more than 2,500 QR).

For the scholarship student:
1. A copy of the ID (the donor is required to be the guarantor).
